Follicular lymphoma (FL) is the second most common malignant lymphoma and usually has an indolent behavior. However, FL remains a highly heterogeneous entity with some patients exhibiting aggressive and chemotherapy-resistant behavior or histological transformation (HT), whereas in other patients the disease is indolent with durable remissions after treatment. The ability to stratify FL patients according to the risk at diagnosis is becoming increasingly important for both predicting the outcome and selecting the therapy.
The aims of the study are: 1) Assessment of tumor genomic alterations (gene mutations, copy number alterations (CNA), and translocations) at diagnosis in FL patients with different clinical situations; 2) Integration of genomic alterations to identify reliable prognostic markers in the different clinical groups; 3) Analysis of the evolution of the disease in patients with paired samples (diagnosis and relapse); 4) Correlate with main clinico-biological features of the lymphoma, response and survival of the patients. Therefore, 104 patients diagnosed between 2012 and 2019 for newly diagnosed FL grades 1, 2 and 3A will be the subjects of this study. We will consider four groups: (1) patients not needing treatment, in observation for >3 years; 2) patients in complete response after immunochemotherapy in whom response duration is longer than 2 years (late progresses or good responders); 3) patients who have relapsed or progressed within the first two years after immunochemotherapy (early progresses or resistant patients); and 4) patients who experienced histological transformation to an aggressive lymphoma at any time. The following techniques will be carried out: 1) histological review and immunohistochemistry; 2) nucleic acid extraction from tumor sample; 3) determination of mutational profile by Next Generation Sequencing and profile of CNA by microarrays.
