HABIT is an Erasmus+ KA220-VET project implemented in Spain, Belgium and Romania that brings together universities, vocational education and training (VET) centres, social services and community organisations. The project responds to increasing challenges such as population ageing, social isolation and fragmented care pathways, recognising that health and wellbeing are shaped not only by clinical care but also by people’s opportunities to participate in meaningful activities, maintain social connections and engage with their communities. Through participatory co-creation, community resource mapping, training and real-life pilot implementation, HABIT aims to strengthen the capacity of professionals and community agents to develop more coordinated responses across health, social and community sectors.

At the core of HABIT is the IMPACT Approach, a person-centred, community-based and participation-oriented framework designed to support people experiencing community disconnection, reduced participation or lack of engagement in meaningful activities. Rather than simply referring people to services or activities, it starts from what matters to the person and connects their goals and meaningful activities with accessible community resources and the coordinated support needed to initiate and sustain participation. Community Care Facilitators play a key role in this process by connecting the person, the community and the support system. In this way, HABIT seeks to foster autonomy, belonging, wellbeing and sustained community participation while contributing to more integrated and person-centred care.
