The BIOMARKID project aims to identify objective biomarkers of diet and lifestyle in children and adolescents using metabolomic and proteomic approaches. These biomarkers could complement traditional dietary assessment methods, which often rely on self-reported information and may be affected by recall errors and misreporting.

As part of the BIOMARKID project, we conducted a systematic review to investigate the association between dietary intake and the metabolome in children and adolescents. Eight studies, including 5,992 participants from 12 countries, were included. Metabolites were measured in blood and/or urine samples using targeted and untargeted metabolomic approaches.

Several metabolites were consistently associated with specific dietary patterns and food groups. Higher adherence to a Mediterranean diet and greater fruit and vegetable intake were associated with higher levels of metabolites such as hippurate, trigonelline/N-methylnicotinic acid, and proline betaine. In contrast, higher consumption of ultra-processed foods was associated with lower levels of branched-chain amino acids (BCAAs), tyrosine, hippurate, and trigonelline/N-methylnicotinic acid. Vegan diets were also associated with lower levels of BCAAs, tyrosine, taurine, and DHA, while fish intake was associated with higher levels of specific phosphatidylcholines.

Overall, these findings suggest that metabolomics can help identify objective biomarkers reflecting dietary intake in children and adolescents. However, further studies are needed to validate these metabolites before they can be used as reliable dietary biomarkers in pediatric populations.
